The consultation form

The only place on this site that takes personal information is the consultation form. What you write in it — your name, a contact number and any short note you choose to add — is stored in this site's own database and given to the centre's clinical team so they can call you.

The form deliberately does not ask for medical history. Health data that is never collected is health data nobody has to protect; the clinical conversation has its own place.

What is recorded automatically

Like any website, the server records the address of the page you open, when you opened it and what browser you used, in technical logs. Those logs exist for error diagnosis and security and are not attached to your name.

Cookies

There are no advertising cookies on this site. The cookies you will see are technical: the language you chose, and the security token that stops a form being submitted on your behalf.

Your rights

You can ask for anything you sent through the form to be deleted. Contact us from the same number or email address.
